Waiver must be accepted by checking the box at the bottom of the waiver for the form to show it as accepted (not just viewed) and analytics event

Requirements

1. Form will not submit until waiver is accepted (unless this is an edit flow and the active waiver is already accepted)
2. User chooses View Waiver
3. User has to scroll through the entire waiver to find the Accept Checkbox
4. User accepts
5. The Accepted Status is updated on the Form
