Please write notes here about pages that are too blurry to transcribe reliably.

Page 3 is blurry, but is workable if you know ITS and MIDAS.

The last line of page 76 isn't good. I see NLV 071 003 in the symbol table, so I think that's it.

@drboone reported page 100 is missing part of the first line. From context, I will guess it's DIRECTORY.

150 is not really readable, everything else seems ok.

the last line (a label?) on page 10 is not quite complete either.
EDIT: according to the symbol listing it would have to be DIEF but with an unknown value (probably -1)
