Haven't touch this project for a quit while. In addition, I haven't used fastify in a while. Therefore, I need some help to update this library in a working state again for newer fastify versions.

An update could also make use of the new esm loader hook api. It is still experimental, but this could be a cleaner approach for this library. It could also fix #11, but I am not sure.
