Take the Code of Conduct out of the CONTRIBUTING.md file

Describe what needs fixing
The Code of Conduct should be in it's own file named CODE_OF_CONDUCT.md instead of along the contributing guidelines.

Why does it needs to be fixed?
The documents should be concise and direct, and while the code of conduct should be referenced in the contributing guidelines, it shouldn't be explicitly there.

Expected behavior
Two documents: CODE_OF_CONDUCT.md and CONTRIBUTING.md with the Code of Conduct and contributing guidelines, respectively. The contributing guidelines should link the code of conduct file for review at some point in it's text.

Comportamento esperado
