Make Locks Keyspace and Table name configurable #12

Keyspace currently defaults to cqlmigrate, table name current defaults to locks.

@adamdougal, I was thinking about this, and maybe just adding a new column to the locks table can work.
The keyspace and table are fixed (this is good), and an extra column can make it configurable per application. This way, would be possible to have different apps working on the same db.
