Bump bl and phantomjs-prebuilt in /openam-ui/openam-ui-ria (#741)

Removes [bl](https://github.com/rvagg/bl). It's no longer used after updating ancestor dependency [phantomjs-prebuilt](https://github.com/Medium/phantomjs). These dependencies need to be updated together.


Removes `bl`

Updates `phantomjs-prebuilt` from 2.1.13 to 2.1.16
