Workflow

File -> Open Image
Edit -> Add Rectangle
drag and drop the rectangle
add name and type (in the head)
File -> Save as JSON
copy the content or save as SVG

With this Form Designer you can upload a Image of a Form and place datafields for further processing. You can export the dataset as JSON or SVG.
