Added

--group-by-style can group two nodes now.
--group-by-style can group into svg now.
defs element is always the first child of an svg now.

Fixed

Groups ungrouping inside a switch element.
Complex groups ungrouping.
--group-by-style recursive processing.
Crash if path has an implicit MoveTo after ClosePath.
--remove-unresolved-classes option had no effect.
